This page explains important patient information for individuals engaging with Apex Vitality. Please read it carefully before booking, purchasing, submitting an intake form, completing an assessment, participating in telehealth, undertaking testing, or commencing any Apex Vitality pathway.

Apex Vitality provides assessment-led health, performance and wellbeing services within a clinically governed framework. Depending on the service, your pathway may involve health screening, intake forms, diagnostic review, pathology or biomarker assessment, telehealth, coaching, care coordination, lifestyle strategy, clinical review, or referral to appropriately qualified healthcare professionals.

Where a service involves clinical care, prescribing, diagnostic interpretation, regulated health services or therapeutic decisions, suitability is determined by an appropriately qualified healthcare professional following individual assessment.

Important safety information

Apex Vitality does not provide emergency medical care, acute crisis support or urgent after-hours care.

If you are experiencing a medical emergency, call 000 immediately or attend the nearest emergency department.

Do not use Apex Vitality’s website, store, forms, email, SMS, telehealth booking system or online platform for urgent medical issues.

Your consent to participate

By engaging with Apex Vitality, you acknowledge and consent to the following, where relevant to the service you choose:

  1. Assessment-led process
    You understand that Apex Vitality services are assessment-led. Completing a form, purchasing a service, booking a consultation or submitting information does not guarantee access to any specific pathway, product, test, supplement, device, medicine, prescription or clinical intervention.

  2. Collection of information
    You consent to Apex Vitality collecting and using information reasonably required to provide or coordinate services. This may include your contact details, health history, symptoms, goals, lifestyle information, medications, allergies, supplements, pathology results, diagnostic information, communications, payment information and other relevant personal or health information.

  3. Clinical review where applicable
    You understand that some services may require review by an appropriately qualified healthcare professional. That professional may request further information, recommend additional testing, refer you to another provider, decline a requested pathway, or determine that a service is not suitable for you.

  4. Telehealth and digital communication
    You consent to receiving services or communications by telehealth, telephone, video, secure platform, email, SMS or other digital channels where appropriate. You understand that digital communication has limitations and may not be suitable for all clinical circumstances.

  5. Testing and third-party providers
    Where relevant, you consent to Apex Vitality coordinating with third-party providers such as pathology providers, diagnostic providers, pharmacies, technology platforms, clinical partners, device providers or other service providers. Third-party providers may require their own consent, terms, privacy policy or clinical process.

  6. Information sharing for care coordination
    You consent to Apex Vitality sharing relevant information with healthcare professionals, clinical partners or service providers involved in your assessment, care coordination, testing, dispensing, supply, monitoring or support, where necessary and appropriate.

  7. Risks, limitations and alternatives
    You understand that health assessments, telehealth, lifestyle strategies, supplements, medicines, devices, diagnostics and clinical interventions may have risks, limitations, side effects, contraindications or alternatives. You should ask questions before proceeding and should not continue with a service if you do not understand the information provided to you.

  8. No guaranteed outcome
    You understand that outcomes vary between individuals. Apex Vitality does not guarantee any particular health, performance, metabolic, hormonal, aesthetic, body-composition, recovery, symptom or wellbeing result.

  9. Your right to withdraw
    You may withdraw consent or stop participating in an Apex Vitality service at any time. Withdrawal may affect our ability to provide, continue or coordinate services for you. Fees, refunds and cancellations are handled in accordance with the applicable Refunds / Cancellations policy and any third-party provider terms.



Your responsibilities as a patient or client

To help Apex Vitality and any involved healthcare professionals assess suitability and support your safety, you agree to:

  • provide complete, accurate and current information;

  • disclose all relevant medical conditions, symptoms, allergies, medications, supplements, prior treatments, family history and risk factors;

  • tell us if your health status changes during a pathway;

  • tell us if you are pregnant, trying to conceive, breastfeeding, planning surgery, under specialist care, or have recently been hospitalised;

  • follow instructions provided by healthcare professionals, pathology providers, pharmacies or diagnostic providers;

  • read labels, warnings, instructions and Consumer Medicine Information where relevant;

  • ask questions before proceeding if anything is unclear;

  • seek urgent medical care when required; and

  • continue routine care with your GP, specialist, pharmacist or usual healthcare team.


Clinical suitability

Apex Vitality may decide that a service, product, test, pathway or intervention is not suitable for you. This may occur for clinical, safety, legal, regulatory, operational or professional reasons.

Where appropriate, you may be advised to seek care from your GP, specialist, pharmacist, allied health practitioner, emergency department or another provider.

Apex Vitality may also pause, modify, decline or cease a service if required for safety, clinical governance, regulatory compliance, incomplete information, non-attendance, inappropriate use of services, or concerns about your wellbeing.

Medicines, supplements and therapeutic goods

No medicine, prescription-only product, supplement, compounded item, therapeutic good, device or clinical intervention is automatically included in any Apex Vitality service unless clearly stated and legally permitted.

Where a medicine or prescription-only item may be relevant, it can only be considered after individual assessment by an appropriately qualified healthcare professional. The final decision to prescribe, not prescribe, dispense, supply, continue, change or cease a medicine rests with the relevant healthcare professional acting within their professional obligations.

You must not start, stop or change any medicine, supplement or treatment based only on website content, store information, general educational material or non-clinical communication.

Telehealth consent

Where your service is delivered by telehealth, you acknowledge that:

  • telehealth may involve video, telephone, online forms, secure messaging, email, SMS or platform-based communication;

  • telehealth may not be appropriate for every concern;

  • technical issues, delays or limitations may occur;

  • a practitioner may decide that in-person care or urgent care is required;

  • you are responsible for ensuring you are in a private, safe and suitable location for any telehealth consultation;

  • you should not participate in a telehealth consultation while driving or in an unsafe environment; and

  • you should provide accurate contact details in case follow-up is required.



Privacy and health information

Apex Vitality handles personal information and health information in accordance with its Privacy Policy and applicable Australian privacy requirements.

Health information is sensitive. Apex Vitality may collect, use, store and disclose your information where reasonably necessary to provide services, coordinate care, manage bookings, process payments, communicate with you, meet legal and regulatory obligations, support clinical governance, and improve service quality.

You may request access to or correction of your personal information in accordance with the Privacy Policy.

Fees, payments and cancellations

Fees, payment terms, refunds, cancellations, rescheduling rules and any third-party provider charges are handled in accordance with Apex Vitality’s Terms & Conditions, Refunds / Cancellations policy, and any applicable service-specific terms.

You are responsible for reviewing the relevant fee and cancellation information before booking, purchasing or commencing a service.

Feedback and concerns

You have the right to ask questions, raise concerns or provide feedback about your care or experience with Apex Vitality.

By booking, purchasing, submitting information, attending a consultation, using an Apex Vitality platform, or participating in an Apex Vitality pathway, you acknowledge that:

  • you have had the opportunity to ask questions;

  • you consent to participate in the relevant service;

  • you understand that services are assessment-led and suitability is not guaranteed;

  • you understand that Apex Vitality does not provide emergency care;

  • you understand that clinical decisions are made by appropriately qualified healthcare professionals where clinical care is involved; and

  • you agree to provide accurate, complete and current information.
